Welcome to the new Clinician Portal

Please take a look through our new clinician portal.

In the menu at the top of the screen you will be able to access:

Resources  – reference documents such as Policy & Procedures, Paitent handouts etc.

Forms  – to download and print (i.e. SPRs, invoice/SAF, consent forms, assessment forms etc.)

Forum – use the forum to post questions you are looking for feedback on. The forum can also be searched to see if someone has already posted a question you might be interested in.

We are looking for your feedback on the new CommuniCare Therapy clinician portal. Use the forums or the “Share your Thoughts” button in the main menu to let us know what you think.

We are still adding new forms and resources daily, if you can’t find a document you use the Portal Questions and Feedback forum to let us know what it missing.
